Facility Information: Nestled within the heart of Tupper Lake, our skilled nursing facility is dedicated to providing residents with top-tier healthcare services and rehabilitation. Responsibilities:

  • Provide physical therapy treatments in accordance with physician's orders.
  • Maintain written evaluations, progress notes, and discharge summaries for residents.
  • Collaborate with other healthcare staff to enhance residents' rehabilitation or maintenance of function.
  • Participate in Resident Care Planning meetings and attend in the absence of the Director of Physical Therapy.
  • Maintain statistical records of residents' treatments.
  • Assist with in-service training programs when requested.
  • Record and update residents' plan of care in the Inter-disciplinary Kardex.
  • Submit requisitions for necessary supplies.
  • Assume the Director of Physical Therapy's duties during absences, including supervising Physical Therapy Aides and handling necessary documentation.

Qualifications:

  • Graduation from an approved school of physical therapy.
  • State license to practice physical therapy.
  • New PT Grads are welcome to apply.
  • Two years of experience in physical therapy preferred.
  • A genuine interest in creating and participating in physical therapy treatment programs for the geriatric and subacute populations.
  • Ability to lead and inspire others.

Qualifications

  • Graduation from an approved school of physical therapy required.
  • State license to practice physical therapy required.
